    Health Screenings

    Routine Screenings (School Level)
The district school nurses conduct the following health screenings throughout the school year: Vision, Color Vision, and Hearing.  These screenings aide in identifying possible barriers to learning.  Those who do not pass the initial screening will be retested.  Parents will be notified and referred for further follow-up only when a problem is identified.  Parents may submit a written request for exemption from any of the listed health screenings.  In accordance with California State Ed Code (Ed Code §49455, §49456, §49452.5), the following screenings are conducted:

    • Vision:  An outside optometrist service screens students in grades TK/K, 2nd, 5th, and 8th; and first year in the California public school system.
District nurses do vision screenings for annual and triennial IEPs.
    • Color Vision:  Grades K or 1, boys only
    • Hearing:  District nurses screen students in grades K or 1, 2, 5, and 8; and first year in the California public school system

    Health Examination
    (Completed by Your Physician)
California law requires that all children have a physical examination within eighteen months before entering 1st Grade.  The Report of Health Examination for School Entry form is available here and at all school sites.

Oral Health Assessment (Completed by Your Dentist)
California law, Education Code §49452.8, requires that children have a dental check-up by May 31st of their first year in public school, at Kindergarten or 1st Grade.  Dental evaluations that have happened within the 12-months prior to school entry also meet this requirement.  The law specifies that the assessment must be done by a licensed dentist, or other licensed or registered dental health professional.  The Oral Health Assessment Form is available here and at all school sites.
